“Councils want to help people live independently in their homes for as long as possible. Evidence has shown that integrating health and social care means people will receive better, more tailored care at home that will help reduce the need for hospital beds.
“Local authorities and their health partners know what is best for their communities. It is vital that we continue to work together with even greater ambition, and alongside the patients themselves, to ensure that people are placed at the heart of the care they receive and any new models of care must reflect this.
“As these plans develop locally, it will be important that councils are fully involved in developing new services to ensure they work best for people and fully join up their care.”
